The first step is to complete a Request for Services. Click HERE for the online form or call 503-253-0964 to do it over the phone. The Request for Services usually takes about 15 minutes to complete.
We take most insurance, including Oregon Health Plan (CareOregon, Health Share). Current wait list for counseling services is four to twelve weeks depending on insurance (on the longer side for Moda, Optum, and non-advantage Medicare). Current wait list for counseling in Spanish is eight to ten weeks.
